The casebook applications has changed! To help your patients access funds for specialty care, please follow the steps outlined below:
Step 1 – Verify
Does the patient have a legal status in the US?
In order to qualify for Casebook or indigent care funding patients are required to be legal residents of Galveston/Brazoria county for >6 months. Some of the immigration status that may qualify for Casebook funding includes Citizenship, Permanent Resident, green card holder, Visa, and and TPS.
If patient does NOT meet this requirement, they will NOT qualify for County Indigent Program or UTMB Charity Care.
Verify that your patient is from Galveston or Brazoria County

Step 2 – Refer to the County Indigent Care Office
Refer your patient to the Galveston Indigent Care Program Application
Under the new process all patient MUST apply for the Indigent Care Program from their before being considered for casebook.
If your patient lives in GALVESTON county:
Hand them an application packet today:
To apply, please ask your patient to fill out that document, collect the required documents and go to the indigent care office at:

If your patient is APPROVED for Indigent Care
If your patient is eligible for indigent care with Galveston, they must register as a Coastal Health & Wellness patient, where they will receive primary care services, and one of their providers may refer them to a specialist for treatment.
If they are approved with Brazoria indigent care program they will need to follow the Brazoria county instructions to access medical services
If your patient is DENIED for Indigent Care
If your patient does not qualify for the Galveston/Brazoria County Indigent Health Care Program, you should ask them to apply for the UTMB Casebook program.
- Patients generally qualify if their income is between 200% to 400% of federal poverty level. Regardless of income, they should apply for UTMB Charity Care Program.
Step 3 – Apply for Casebook
Place a Casebook Epic Referral and answer all required questions
In order for your patient to be able to apply for casebook you must submit an Epic Casebook referral and complete all the required questions. After this order is placed, your patient MUST call the financial services office at UTMB to continue their application.
Place a casebook referral on Epic for the patient and answer all the required fields:


Place all required Epic Referrals for the patient
Place all the referrals needed for the patient’s care. Please ask the patient not to schedule any appointments until their casebook is approved as they will be responsible to cover the cost of the visit.
Hand your patient a copy of the following document and ask them to call the UTMB financial counseling department.

If you have completed all of the above steps, you are done assisting your patient with their Casebook application!
